- Report all incidents (accidents & near misses) to your immediate supervisor asap
- The incident will be investigated so that due to causes corrective measures can be implemented to prevent any recurrence
- Any lessons learned will be formulated and distributed
- Use the Butendiek Incident form (this can be received from the HSE department via your supervisor)
